The Laurandy Centre is seeking an experienced and motivated Care Planning Coordinator to join our dedicated team.
The Laurandy Centre is a registered day support service for older people, delivered by a respected local third sector organisation and funded as a commissioned NHS service. We have built an excellent reputation within our community for providing compassionate, high-quality support that promotes dignity, independence, wellbeing, and meaningful engagement.
About the Role
Working closely with the Centre Manager, you will play a key role in ensuring that our care and support planning processes remain robust, person-centred, and compliant with current legislation, standards, and best practice.
Your responsibilities will include:
• Developing, maintaining, and reviewing care plans and risk assessments.
• Supporting the delivery of high-quality, person-centred care and support.
• Monitoring and auditing care documentation to ensure accuracy and compliance.
• Promoting positive outcomes, safety, dignity, and wellbeing for service users.
• Supporting quality assurance and continuous improvement initiatives.
• Liaising effectively with families, health professionals, and external agencies.
• Assisting with the promotion and development of the service within the local community.
About You
You will have:
✔ SVQ Level 4 (SCQF Level 9) in Social Services and Healthcare, Leadership and Management, or an equivalent qualification.
✔ Experience of working in a senior care or supervisory role within health or social care.
✔ Excellent knowledge of person-centred practice, care planning, and risk assessment processes.
✔ Strong organisational and communication skills.
✔ The ability to work independently, prioritise workload, and use initiative.
✔ A commitment to continuous improvement and delivering the highest standards of care and support.
